DR. ERWIN MAURICE PATLAK passed away on December 6 in Miami. U. S. Army Surgical Technician who liberated the Philippines, University of Illinois graduate psychiatrist physician served at Lutheran General and Grant Hospitals. Led Northbrook Rotary Club and North Suburban Y. Left his beloved Chicago for the Bahamas as a Medical Officer and headed Onslow County Mental Health near USMC Camp Lejeune. After loss of wife Helena in 2012, survived by eight children: David, Rachel (Happ), Joshua, Jonathan, Daniel, Susan (Kenyon), Rebekah (Kohles), and Sarah with 14 grandchildren and 11 great grandkids. Funeral 12:15 p.m. on Tuesday, December 10th at Shalom Memorial Park, 1700 W. Rand Road, Arlington Heights.
